5. Procedure for Responding to Requests for Disclosure, etc. After verifying identity, we will mail a “Personal Information Disclosure Request Form” to the individual requesting disclosure. Upon receiving the returned “Personal Information Disclosure Request Form” from the requesting individual, we will promptly respond to the disclosure request without delay. Responses will be provided either by telephone or by mailing a “Personal Information Disclosure Response Form” addressing the request. Please note that we do not charge any fees for processing disclosure requests.
